Qualifying second or other owner, must officially transfer warranty or file boat registration in his/her name prior to tournament win.

I AGREE TO THE FOLLOWING TERMS:

I own a Falcon Boat and it is/will be registered with Falcon Boats.

I will meet all participating guidelines of the Falcon Boats High Flyers program for each sanctioned tournament.

I will meet the clothing requirements listed and wear required apparel during all competition days.

I understand that Falcon Boats’ sole decision is FINAL in all matters of eligibility and approval of claims.

I understand that it will take 4 – 6 weeks for processing and payment on all qualified and approved claims.

I understand that Falcon Boats will file a Miscellaneous Income Form 1099, required by the IRS, for each approved claim.

I agree to allow the use of my photos, name, and/or likeness for advertising and publicity purposes without further compensation in any form, as a participant in the High Flyers program.

I will complete, sign, and return this form along with $100 enrollment fee to Falcon Boats.

By signing and submitting this form and entry fee, I agree to and will abide by the terms and conditions of the Falcon Boats High Flyers contingency program as set forth by Falcon Boats, LLC. Falsifying documents is a criminal offense and may result in prosecution.
